3 Easy Steps for Open Enrollment:

Step 1 - Create a new account in our online Parent Portal.

Step 2 - Once logged in to the Parent Portal complete an 'Application for Enrollment'.

Step 3 - Download, complete, and then fax or mail the form labeled 'Download Signature Pages', also located in the Parent Portal.

After submission, your enrollment application will be reviewed, and an enrollment specialist will contact you.
